Star Princess - MAIB Update Following Fire Onboard March 23, 2006

The MAIB issued a Safety Bulletin following the fire which broke out on board the Bermudan registered cruise ship Star Princess, whilst on passage from Grand Cayman to Montego Bay, Jamaica, on 23 March 2006.

The Safety Bulletin contained 2 essential recommendations to cruise lines and operators of passenger vessels:

1.

Immediate actions to counter the risk of a recurrence of such an incident.
2.

Actions to provide a permanent solution to prevent such an incident in the future.

At 20 June 2006 the MAIB had received positive assurance from the following cruise companies/operators that they were adopting measures to comply with the above recommendations, within the timescale prescribed:

Star Princess - fire regulations for balconies agreed
The Maritime Safety Committee (MSC) at its 81st session in May 2006 approved draft amendments to SOLAS chapter II-2 and the FSS Code to strengthen the fire protection arrangements in relation to cabin balconies on passenger vessels, in the wake of the fire aboard the cruise ship Star Princess.

The fire in March 2006 aboard the Bermuda-registered cruise ship Star Princess, while on passage between Grand Cayman and Montego Bay, Jamaica, began on an external balcony and spread over several decks. The cause is currently being investigated by the UK's Marine Accident Investigation Branch (MAIB) on behalf of the Bermuda Maritime Administration in co-operation with United States' authorities. Although the investigation is not yet complete, the MAIB and the International Council of Cruise Lines (ICCL) issued a safety bulletin and a safety notice, respectively, which include urgent safety recommendations pertaining to the incident and a related paper was submitted to the MSC by the United Kingdom, which the Committee agreed to fast track.

The proposed draft amendments to SOLAS chapter II-2 are aimed at ensuring that existing regulations 4.4 (Primary deck coverings), 5.3.1.2 (Ceilings and linings), 5.3.2 (Use of combustible materials), 6 (Smoke generation potential and toxicity) are also applied to cabin balconies on new passenger ships.

For existing passenger ships, the MSC approved relevant provisions to require that furniture on cabin balconies be of restricted fire risk unless fixed water spraying systems, fixed fire detection and fire alarm systems are fitted and that partitions separating balconies be constructed of non combustible materials, similar to the provisions for new passenger ships.

The draft amendments will be circulated with a view to their adoption at MSC 82 in November-December 2006.

It was agreed that the Sub-Committee on Fire Protection (FP) should review the fire safety of external areas on passenger ships and develop draft guidance for the approval of fixed water-spraying, fire detection and fire alarm systems for cabin balconies, taking into account that some existing passenger ships are already installing such systems in response to the Star Princess fire.

The MSC in the meantime agreed an MSC circular on Operational recommendations for passenger ships with cabin balconies. The circular recommends that the shipping industry implement a number of recommendations including increased vigilance such as the deployment of lookouts, fire patrols and television surveillance systems; passengers and crew should be advised not to leave towels and personal belongings on balconies; and passengers and crew should be reminded of the hazards associated with the use of unauthorized heating elements such as electrical heating coils used in cups or mugs and open flames such as candles.

MAIB's report of its investigation of the fire aboard the cruise ship Star Princess has been sent out for consultation.

Under the provisions of the Merchant Shipping (Accident Reporting and Investigation) Regulations 2005, a 28 day period is required to allow those involved to comment on the report.

Publication, both on this website (MAIB) and in hard copy, is scheduled for mid-November.
Basically that means they have discovered the cause, decided on changes that will need to be made to the ships (if any) in relation to materials/design/safety features and have passed the report to the cruise lines and their governing bodies for evaluation and for work to be carried out where necessary.

When the full report appears, I shall post a link to it on this thread. Looking at the length of time this has taken, and going by air accident reports which I receive, I am guessing there will be alot of changes and possibly a few procedures changed as a direct result of this tragic fire.

Some changes will, inevitably, cause some people to complain, but whatever comes from this report has to be accepted as steps to improve the safety and well being of cruise passengers....they might inconvenience but they will potentially save your life.

The Marine Accident Investigation Branch (MAIB) published today its report
into the fire that broke out on a cabin balcony on board the Bermuda-registered
cruise liner Star Princess on 23 March 2006.

The investigation was carried out by the MAIB on behalf of the Bermuda
Administration with the help of the United States Coast Guard (USCG) and
the US's National Transportation Safety Board (NTSB).

The Chief Inspector of Marine Accidents, Stephen Meyer, said:

"This investigation is probably the biggest conducted by the MAIB since the
Sea Empress in 1996. It has already had a significant impact on both the
cruise industry and the International Maritime Organisation (IMO) regulation
relating to cruise ship balconies.

"In the seven months since the incident, we have been working closely
with the IMO, Princess Cruise Lines and its parent company, Carnival, and
representatives of the cruise industry to understand how the fire spread
and to introduce measures to prevent similar incidents happening in the future.

"I must congratulate the industry on the speed with which action has been
taken.

"Carnival began replacing all combustible dividers on the 26,400 balconies
of its 81 vessels immediately following the incident and expects to replace
all of them with non-combustible ones by the end of this year. This is a
major achievement.

"IMO has already issued interim operational recommendations to cruise lines
with balconied vessels and has proposed amendments to the Safety Of Life At
Sea Convention (SOLAS) which will be considered for world-wide adoption as
early as next month.

"This is well ahead of IMO's normal timescale for introducing new safety
measures and recognizes, I believe, the thoroughness of the MAIB's
investigation and the importance of the recommendations we have made.

"In our discussions with the industry, I have been concerned that the
measures now being put in place will improve significantly the safety of
cruise ships. I am satisfied that this is the case."

The IMO's proposed amendments relate to the use of non-combustible dividers on
cabin balconies, and the restricted use of combustible materials on balconies,
unless fire detection and extinguishing systems are fitted.

The report is a very interesting read...if a bit on the long side. There are some photographs too, some of which are quite hard to look at purely for subject reasons. Hopefully all the recommendations will be taken up by the lines after this accident...which the MAIB are certain did start with a discarded ciggy.

ARLINGTON, Va. (Oct. 23, 2006) - The United Kingdom’s Maritime Accident Investigation Branch (MAIB) today released the final report from its investigation of the fire which occurred on Star Princess on March 23, 2006. According to MAIB, the fire was determined to have started on a balcony and spread due to the presence of combustible materials on balconies.

The International Council of Cruise Lines (ICCL) has worked closely with investigative and regulatory agencies to review current policies to enhance safety for passengers and crew. Ensuring the safety and security of its passengers, crew and vessels continues to be the ICCL’s highest priority.

The ICCL is pleased that MAIB incorporated all of the recommendations which were issued to its membership following the fire. Of the immediate actions included in the recommendations, cruise lines:

• Increased vigilance on vessels with balconies, with the dedicated lookouts and additional fire patrols;

• Reviewed crew onboard training and response to ensure firefighting teams are prepared for a fire occurring in the balcony area; and

• Advised passengers and crew on proper fire safety protocols for balconies.

As part of the long-term actions, ICCL member lines are in the process of replacing balcony dividers with non-combustible materials.

“The industry is pleased that MAIB’s final report commends the proactive measures taken by our member lines,” said J. Michael Crye, ICCL president. “The dedication of our members to complete the recommendations in a timely manner demonstrates their serious commitment to safety.”

The cruise industry will also continue its participation in meetings at the International Maritime Organization to adopt additional fire protections to the International Convention for the Safety of Life at Sea (SOLAS). These changes to SOLAS should be adopted at the December meetings of the IMO Marine Safety Committee.

We acknowledge the report issued today by the United Kingdom's Marine Accident Investigation Branch (MAIB) regarding the fire aboard Star Princess which occurred on March 23, 2006, and can confirm that we have fully cooperated with the inquiry into the incident.

We remain, as from the first day, devastated that the fire occurred and we wish to again express our deepest sympathy to the family of the passenger who passed away, and to our other passengers and crew who were affected.

Immediately following the fire, and during the past seven months, we have striven to do everything in our power to ensure a similar incident could never happen again.

In our opinion, the report issued today provides a professional and balanced overview of the various aspects of the incident. It points out the many safeguards and procedures Princess had in place to protect passengers, and it confirms the professionalism demonstrated by our crew members, to whom we are very grateful.

However, we also acknowledge that the report shows there were lessons to be learned by us, and that some aspects of our response to the fire, and related training, needed to be addressed. We can confirm that we immediately took corrective action and implemented procedures which addressed the shortcomings detailed in the report. Today, there are no recommendations made by MAIB that Princess has not met.

I am not sure tbh, Chuck. As they operate ships out of the UK, I would imagine that they would have done so by now, even if they haven't publicly confirmed it. The MAIB would, I would have thought, taken a very dim view of their ships had they not complied.
